I think this is a question to take up with the doctor who prescribes the medicine, though you should also get an information sheet that describes the possible side-effects on it. Girls playing with trains and cars may be unusual but it's not unknown (I used to love them myself). However, if she is excessively focused on only one kind of play and has trouble negotiating play with other kids, you might have a professional look into it. It could be a sign of Aspergers, it could be just lac of skill at being social, or it could be that she's just very advanced. A professional who can meet her and watch her interacting will be able to give you an assessment. Her size does sound on the low side, so please go to a pediatrician and have that looked into.

Hi Cathy,
I definitely would talk to her doctor about the meds. A lot of it does sound like personal issues, self-esteem issues and the like.
Do see her doctor about her stature, and you may want to talk to her about her avoiding others issues as well.
Best,
Clyde
